Reporting Patient Harm

This survey takes about 3–5 minutes for most reports, a little longer if the harm was serious. Here's what to expect:

You'll be asked who you are, roughly what happened and where, and how serious the outcome was. Depending on your answer, you'll either get a couple of quick follow-up questions or a few more β€” we only ask for as much detail as your situation actually needs.

Your name and email are optional throughout. You can report fully confidentially. The only thing we do ask you to confirm is that we can use what you share β€” without identifying you β€” to help build a clearer, evidence-based picture of what's happening across the country.

If what you're reporting involved hospitalisation or ongoing harm, please also consider reporting it to the AHPRA, HCCC or your state health complaints body β€” this survey helps our advocacy work, but it isn't a substitute for those formal channels.
